501(C) 3 Umbrella Status Annual Renewal

As a 501C3 club under the Federation umbrella, you are subject to general supervision by the Federation and required to submit an annual financial report with any address or contact changes by October 1 of each year.

To renew your 501C3 umbrella status with the Federation you must:

  1. Already be under the Federation umbrella.
  2. Fill in and submit the renewal form on the reverse side with all financial and club data as requested.
  3. You must file an information report (990N/e-postcard) with the IRS before November 15, if your club is under $25,000 of total revenue. If your club’s gross revenue from projects, donations, dues, etc., is over $25,000, you must file IRS form 990-EZ with the IRS and send a copy of this with your Federation renewal form.
  4. No filing fee is necessary to renew.
  5. Clubs not renewing by the October 1 deadline will be dropped from the umbrella and will need to reapply and pay the fee for umbrella status.
  6. A reminder that clubs are required to file an information return (Form PC) with the Attorney General’s Office by November 15 of each year as well.

All fields must be completed. If  the club has no revenue or no expenses for a particular category, enter 0. Values should be entered as either whole numbers (xxxx) or with two decimal places (xxxx.xx). Please do not use comma separators.















The form is due Oct 1 and should agree with information returns filed
with State and IRS on Nov 15.
